Prior to your initial intake and evaluation with Dr. Randall, please print out and complete the highlighted parts of the following forms. These forms will be necessary during your first appointment with Dr. Randall.
Separate "Release of Information" forms will need to be completed for the patient's Primary Care Physician, the patient's designated Emergency Contact, and if applicable, their current Psychiatric provider.
